If you could press a button that had a 60% chance of sending you, with all your memories, back into your younger self's body back when you were half your present age; and a 40% chance of just erasing all your memory and not sending you back in time... would you push it

I don't think I would push it... I'd prefer my younger self to retain that innocence I had when I was 11-12, and I'm not terribly unhappy with any of the choices I've made in my life thus far to want to possible have prior knowledge in order to change things. Unless I could maybe affect something much bigger than myself (I don't know exactly what) I definitely don't think I would push it!
