I use to LOVE those things.
I dont remember them ever having a name though. I think they were always just called fortune tellers =-/

Hey.. the boys called them cootie catchers!!! Ha ha.. do you remember that? You can make customized ones here for your friends.. send it to them. Maybe they know the name of it. ..

They were called either Fortune Tellers or Cootie Catchers.
