it's more than likely explainable by the population of "the federation" which is - i presume - enormous... ergo, the starfleet officers will be selected based upon merit, and, as the sample population is so big, they will be the best of the class.

Maybe to have a starfleet exemplory record and finish top of your class you have to blow every other student's ship out of the water and murder everyone else, so there is no-one who lives long enough to acquire a blemish on their record
