That's plausible, especially if he seemed to lose congnitive ability and undergo behavioural changes in a short period of time (I don't know offhand if that is so).

More numerous very small strokes are also possible.

If he's had a stroke, that would be clinically significant and in his medical records.

…which would mean his doctor outright lied when he signed the bit of paper saying Trump was in good health. It feels like that would be a step too far for most medical professionals.
