It's like the old times: they're in Paris, holding hands and admiring the view. Everything is the same, except him: Edwardian suit, hair in gentle waves instead of little curls, softer features. Romana doesn't seem to mind - why should she? Didn't she once regenerated, several times, out of desire to change her appearance? She leads and he follows, laughing when he should be in pain, following a woman that shouldn't be there.
"I have to go," he says finally, trying to stop, never letting go of her hand. Romana turns her head around a little, still running.
"Don't," she says. "Stay with me a little longer."
And the Doctor does that, thinking of the good times they had together, even as the city starts falling apart behind them. He thinks of San Francisco, Romana might like it. They could go there sometime.
If only Romana were truly there.
If only the Doctor weren't dying. And this time, it might be final.
His skin starts to blister and his suit starts to fall in burning tatters. He can feel his life slipping away. Behind them, Paris is morphing, from the City of Life into ruins, and the ruins into the burning Gallifrey, a dying planet, his home and his people gone before their time.
There's no way to go but behind blond bouncy hair, a promise of good memories no matter what. So he keeps running because this fragment of a dying Time Lord's imagination refuses to stop and let him die.
She finally stops and turns to face him. When he reaches out to touch her, his hand is glowing and a new life courses through him. He's not dying for good after all. Romana smiles even as she fades away. She's leaving, he killed her, he killed them all. And yet she smiles. Even in death, she carries the memories of what they once were. Of what he might become one day.
And the Doctor smiles back and takes this last memory with him as his body regenerates once again.
