I too think about her every now and then. The only thing I can say for myself is that at the time I was experiencing my own downward spiral, and didn't recognize that fact that my friend was trapped in one of her own. I fought against the current, she was sucked under. Don't blame yourself for what happened, we were all too stupid to realize what was going on, and I think she hid it well. It's in the past. You can always try and talk to her about it, though I don't think much will come of it.
