Simply enough for the dressing-room event make your application as follows:

Contact: (Most of us use AIM, but as long as you list some way we can contact you, it's fine!)
Age: (Your age.)

Character: (What character are you going to be playing?)
Age: (Character age.)
Series: (What series are you from? Tales of the World: Radiant Mythology? Tales of Destiny? Put that here.)
Status: (put if it's just a normal version, AU version, Timeskip, series switch, etc here.)
Basics about your character: (Either a link, or something that will tell us about your character, so we know how to interact with them better. This will help you for CR as much as it helps us and the other players for clarity.)
