As someone how did the seamless ski sweater for an adult (my husband), I have to say that you just have to wing it for those gaps. They are much bigger in the adult size, and I just kind of weaved and picked up stitches and weaved some more kind of thing. I was still playing with it when I finished it. Now after more than a year of wear and several washings, you can't even tell what I did.
Try duplicate stitch before and after actual grafting. Also, when knitting top down you have no need in grafting (just reverse all directions for bottom up raglan) -- but you may have need in twisting of edge stitches to prevent gaps.
