Well I've seen a few documentaries (the titles slip my memory at the moment) on food talk about it. Where cows shot up with steroids/growth hormones produce so much milk their over-sized udders begin to drag on the floor creating lesions. Eventually puss starts to form and sometimes this puss can mix with the milk during the milking process. I'm not certain how often this happens nowadays but it's said that was happening to cows who were given rBGH growth hormones.
As for the shit in the meat, there have been cases where during slaughter if the intestines are accidentally cut that it's possible for feces to come in contact with the meat that's being used for consumption. I'm sure this doesn't happen all the time but I wouldn't be surprised if it did.
