As to Bagoas being surprised that Alexander looked right through him, my initial thought was: well, the book is from Bagoas' perspective BUT by the time that Hephaistion died he had already known A&H as an 'institution' for a good couple of years. So really, he should have had full knowledge of what they meant to each other. So he shouldn't have expected to be any sort of emotional crutch to Alexander.
