I don't think dreams die, Tisamenus. I think dreams are rather malleable, and as such, when they are battered by the cruel fist of reality, they reshape themselves into something better equipped to handling the situation. That is, if those dreams are ones you truly value. The dreams you truly value are ones that do not die all that easily.

I think that's part of what the phrase means. Dreams do not die easily, they die hard. It takes a lot to make them die and even when they do, we still clutch onto them. But like dust, they begin to slip through our fingers.
Not if you hold tightly. There will always be dust coating your hand, even if you think the last bit of your dream has fallen completely through your fingers. It's only when you wash your hands of it that you truly lose that dream.
Keep your chin up. Things may not work out exactly as you wish them to, but perhaps there is something better in store. Have hope.
