When was the last time you saw a doctor about this? I don't want to scare you, but my mother was like that when I was a kid. She used to buy those super overnight, looks more like a diaper pads and had to wear multiple ones at a time and also had the bleeding like that for months on end thing. It ended up being cancer and she had a complete hystorectomy at the age of 39. Seriously, nag your doctor if he or she isn't doing anything about it because that is far from good. Even if it's not the same as my mother, you're still losing far too much blood.
If you're bleeding that much, that fast, HOSPITAL. Seriously. The Mirena may be failing in terms of controlling your cycles and all OR it may have expelled. Skip your Obgyn's office and go right for the hospital. Worse that happens, they say "oh, that happens to some women on Mirena" and send you to your regular doc. Best that happens? they FIX THIS SHIT. :( *frets*
Honey, I agree with the other two commentors. Medical attention - NOW! Do not pass go, do not collect $200, Go.To.The.Doctor. If that is the ER, fine. If your OB can see you NOW, fine. This is past the point of slightly worrying to DOOD! MEDICAL ATTENTION NOW!

A friend of mine who had a lot of the same issues you're having, even with the Mirena, thinks she may have found an answer in this condition: Congenital Adrenal Hyperplasia. She heard about it on Medical Mysteries and it just seemed to describe her.
