There are two things that could happen with it, I guess. Either he could revert to forever waifu mode and never make that transition in personality, or-- or, he could start being an angry sort of emo kid. Considering when he met Sadiq it was at a point where he'd finally broken off fall contact with his dad. He'd be all /RAGEQUIT at life.
Sadiq...wouldn't change at all though, which makes me feel bad. I can see him being a lot more angry emo kid - that seems more PLAUSIBLE - and he doesn't make any kind of personality transition until Arthur enters the picture, but that also changes the nature of Afonso and Art's relationship too, now doesn't it?
Romano in part does hang out with Tonio a lot because he is aware of the feelings and is of the opinion that "if I can't give him what he wants I can at least give him something close."
Then again it also might result in a less protective nature on Antonio's behalf - dating wise but also in regards to other events like Romano's kidnapping it. It would be easier to let Romano go if that bond wasn't there and he can, in some cold hearted way, be written off as expendable.

It would have just been her and Hiram and Affie. And there wouldn't have been any of this mafia involvement at all. They would've moved to the countryside or something and Affie would have been a teacher and never met Arthur. (Unless you believe in fate in which case they might have met serendipitously)
