Its a really really good photo. The hand in the foreground brings the viewer into the photo. I noticed that you can actually see the blood on the back of the tub in the reflection. The stark white of the wall and the back of the tub is a little distracting to the composition. You might consider cropping the image down so all you see is the water. Let the reflection of the blood on the edge of the tub speak for it self.

Anyway I think this is a great outlet. Maybe making it real (in a sence) will help it go away. nice hospital gown by the way. (not)

My only criticism is that you might want to use fake blood next time :P
