Sam explained it succinctly, "Every member of my family was dead. I was truly alone." Sam thought Dean was dead and presumingly gone to heaven. Remember when Dean yelled at ghost Bobby for sticking around instead of going to heaven?
Dean hiding Benny's existence from Sam, that's not going to turn out well. So I guess hell makes you emo and purgatory turns you into T-1000.
The current theory out there is Dean and/or Benny killed Cas to power the exit out of purgatory. If Cas willing sacrificed himself I think Dean would have told Sam, but instead he was evasive when Sam asked for an absolute confirmation.
Finally, oh my was Sammy ever gorgeous! I almost can't believe the network had Sam's hair grow that long. And Jared looked so young in this episode. I thought fatherhood and sleep deprive are supposed to age you.
