transgender homeless teens? doesnt that require an operation that requires money?

1) what TEEN can afford such an operation?
2) what HOMELESS person can afford such an operation?
3) what HOMELESS TEEN can afford such an operation?

This really confuses me. Unless they are using a different definition of transgendered than I am.

Transgendered / trans-sexual also is termed for those who -would- do the surgery, or choose to live life as the opposite sex. Not just post-ops.
