Don't yell chirurgeon at all. Anyone with a first aid card can be a chirugeon.
Some of them are talented and have common sense. Yell specific names.
"Temair" "Hawk" "Svan" "Selena" "Adonna" work nicely, at least those are the ones I use.

You can always refuse all or part of care from anyone on the planet unless you are unconscious.
