Haha, good eye--that's my favorite part too! I wondered if anyone would catch that. It's Elliott and I think he's wearing his funny little homemade Nation of Islam style hat. It makes a funny silhouette, reminds me of Kid N Play.

The tooth was all up in another tooth's business. From my count, they used several types of buzzy saws, a drill and a burr grinder. And shiny pliers. My mouth hates me right now.
