I'm with you on finding this insanely frustrating. I rented a car about once a week for two years and ended up just winging it on insurance. In retrospect, I think this was idiotic, but in practice, I'm pretty sure I'd do the same if I were doing that still, and on the rare occasions that I rent a car, I probably will. I do have a credit card that's supposed to cover some of this, which makes me feel not completely irresponsible...
These days, though, I've been using zipcar, which includes insurance for its drivers. It's more expensive per day than renting, but it's also a nifty business model, so I feel good about it, in general. In practice, now that I've quit the job that required me to have a vehicle once a week, I'm not going to be hassling with any of this - yay!
