OK- I have 8 week old twins and I have a bunch of feeding related questions currently. Here's a quick background. My 23 month old DS had reflux issues as well as dairy and soy sensitivities when I breast fed him

How long has it been since you eliminated the dairy/soy? I ask because the proteins can stick around in your system for a while, so it could be a week or more before you see improvement if he's very sensitive.
They're three weeks adjusted, then? It's entirely possible that they'd be going through a growth spurt now -- I've seen babies follow their actual age for those, or their adjusted age... or some thoughtful babies who follow both, aren't they thorough? :)
I don't think it sounds like you have a supply issue, though -- no more than every woman has a bit of a supply 'issue' in the evenings, when the natural hormonal cycle tends to dip a bit. Because his distress happens after a feeding, it does sound like it's related to the feeding itself, and may improve if he is also getting the reflux meds (definitely discuss w/ your dr.) I would absolutely keep up with the no dairy/soy diet, as well, since any reflux-y behavior would be worsened by him eating something that disagreed with him.
It's been over a week since I eliminated the soy and about 2 since I eliminated the dairy. I didn't eliminate it entirely though (since I never had to with my first DS)- if there is butter or something small cooked into something I still eat it. So I wonder if I'm just going to have to be more regimented with him? I think I will see if he improves on the reflux meds, so I can make sure I know which thing is making the difference.
you son sounds like he has similar issues to what my dd has. you will probably have to eliminate all dairy and soy. when i used to have the tiniest bit of dairy, my dd would scream uncontrollably like your son. i now scrutinize every ingred label bc things i wouldn't suspect have dairy do- cassein, lactic acid or some ingreds that pop up in things you would think don't have dairy like lunchmeat.
your son may, also, be a high needs baby. eliminating dairy/soy has helped her temperament a lot, but she still can get into crying fits and needs to held a lot of the time. her temperament did calm down a lot after about 4 months, though. before then, she was attached to me 24/7.
