I *do* know where you're coming from though I'm not going to play, "Can You Top This." However, can I share a resource with you that helped me a lot?
First Things First, by Stephen Covey is a book that I'm convinced saved my life. It's available at any library and I'd really encourage you to give it a look.
Actual mileage may vary of course, but it was a real help to me.

Sometimes you just have to jump to see what happens, but I know what it feels like to otherwise be 'responsible' and not be able to do that for fear of fucking up big time.
