Does the image we use for the base have to be of only the Beatle wife/girlfriend, or can it be of them with their husband/boyfriend?

Doesn't matter. Just remember you can only use their wives though.

That's what I mean - If I choose to use an image of let's say John/Yoko, I'd have to crop John out, right? Or could I leave him in, to enhance the lyric line I want to use, just have him less of a focus in the icon?

For that, it doesn't matter. You can have who ever you want in the picture just as long a Beatle wife is present :)
