I would personally go with your first idea because it sounds like something I would choose to read and I think has the most leeway, but that is my choice.
The second one I think would be the worst because it is the most unrealistic and I don't know how any doctor could declare a person who claims to hear voices sane, or if he was sane, it would be more likely that he would be hailed as a liar than a prophet. (I could just be overestimating the mind of the general population.)
The third one, I think, sounds like something my friend's mom would give to my friend's little sister to remind her to always pray and love God.

Some kind of combination of the first two could work quite well though... like, first of all people think he has schizophrenia, and he goes through a lot of Hell over it, except some people do believe him and in the end it turns out he's sane...?
I dunno, I'm rubbish with plots too!

As others said, a mixture of the first two ideas would work well. I'm actually writing about a utopia as well, this year!
