Um, sounds to me like Program B is the best choice on multiple levels. I am not sure how to turn other programs down: perhaps send them a nice letter saying that while you loved their program, you are not interested in moving there for five years of your life. that isn't an inconsequential period of time....
you sound like youre leaning towards program c a little too- as someone in the process of applying for a phd program im interested in this. also, practical question: im curious as to why, if youre goal is to teach, with tenure track, why you seem to shy away from ta-ing? im doing the full on 3-classes-of-25-students-all-alone thing, and its more reewarding than the rest of school right now. just something to consider. also, congrats!
