No one can understand and care for one particular person wholeheartedly; they'd have other people to look out for, themselves included.
But that doesn't mean no one cares or understands. Sometimes it's your perception, whether someone TRULY cares or not. There may be people who care but are forgotten by you, and likewise, there are others whom you notice, but don't care.
Coincidentally, but unfortunately, those are the people who matter to you.
