A good recent book on what ails the American health care system is "A Second Opinion" by Arnold Relman, a professor from Harvard med. school and chief editor of the NEJM. He does a good explaining of explaining why the free market has done such a lousy job with health care and why commercializing medicine decreases quality, increases cost and (less surprisingly) decreases equity.

I should clarify. He's an emeritus professor and *former* editor in chief.
