Not enough information to answer the last two! It depends so much on the project and the engineers involved and the house style guidelines.

Not really. It's just that in my experience C++ projects are less pleasant to maintain / debug than C, while C++ has some attractive features that means it's tempting to *start* a project in C++. Just wondering if that's just my opinion.
