I've got CT's autograph and a picture with her from when she did Under the Blue Sky - but there was no DT involvement, so things were a lot quieter.
Sometimes, as shows get later in their runs, things calm down a bit - but this is David Tennant we're talking about, so I think it's unlikely that will happen.
We check out where the stage door is beforehand and work out the shortest route - and then cross our fingers!
Well, like I said before, the fact that they usually come out pretty quickly and then go back inside rather than waiting until they're completely ready to head off home (!) speaks volumes. They know how popular they are, and they know it's part of the job, so they're still "on" so to speak. I'm sure some nights they might not feel like facing the masses, but they do it because its part of the deal. And you'd never guess which are their off nights :)

I reckon if you do do a recon the night before, you might want to take something to get them to sign, just in case. In fact you might even be able to get in there ahead of the crowd since you won't have been in the theatre.
It does sound, from two people I know who went, that the actors also sometimes properly leave from the front - not that I'm suggesting accosting them there, but maybe it'll make you feel better to know that they aren't necessarily trying to go out the way that the crowd is, necessarily.
