Given that the whole song is about an all-consuming fire that is apparently unstoppable (ie, "This fire is out of control/it's gonna burn this city"), maybe Alex Kapranos is simply referring to the fact that it continues to burn and is indeed very painful. This is also suggested by the delivery of said lines.
