Tolerance build up and so forth it's actually possible.
We were just talking about this the other night w/ a friend who's a nurse. He has a patient who's on what *should* be a nearly lethal dose of morphine a few times a day. (Car accident, pain killer, long story) However, she's an attorney and she functions just fine with no strange effect. You'd never know she took anything.
If the guy built up his tolerance to it he can be popping a few an hour and nobody may ever notice.
