A seriously depressing list, with only two points that I'd possibly disagree with. I've seen some people hover on the edge of a conversation for 30 mins becasue they're very shy and hope someone will talk to them. They want to feel involved, but can't pluck up the courage to interrupt.
I wash my feet a lot more often than my shoes, so I feel my feet are actually more hygienic in a cafe.
I think the points about respect for religion, sexuality, etc are all too valid. There does seem to be a case that the more liberal people feel they are, the more intolerant they can sometimes be. (a vocal minority)
Yeah, I think the "no shirt no shoes no service" is a particularly American thing.
The other one I'd maybe disagree with (on a case by case basis) is the "don't pet a service dog" thing - I'd add "without permission" to the first one. I mean, I'm assuming the OP doesn't want people to not talk to folks who need a service dog, but it'd be... I dunno what the word is, not "rude" but just weird somehow (as a dog person) to not include the dog in the socialisation with the person. I.e. if you say hello to the person and chat for a minute, of course you should say hello to the dog and, ideally, if there are no objections from person or dog, give it a pat
