if you look at the other d&d podcast, not from the penny arcade guys, watch the paragon adventure one. it was really interesting.

conserving them isn't an option - bad guys are beefed up in 4th ed, with the exception of minions. we got worked over by a couple teams of kobolds. one is frequently faced with either using a healing surge or keeling over.
we do have a cleric in the party, btw. but that just enables you to use a healing surge in combat, with a few bonus HP added.
