not that i'm recommending it or anything, but there's an option to find a doctor willing to say you need the orthognathic surgery to correct acute obstructive sleep apnea, not related to TMJ... :/
my sister works in the medical field (not dental medicine, however) and she tells me about all the insurance horror stories--if it makes you feel any better (not that it would), the doctors get screwed by them, too.
I've got Medicaid and Medipass (the dental equivalent) and a list of about 50 local dentists on my desk. I'm down to my last 10 numbers: the doctors with Hispanic last names won't accept it, the doctors with European last names haven't heard of it and respond with contempt when I explain it, and the offices that DO accept it won't take any new patients. I've got cavities that need filling, wisdom teeth that need removing, and a TMJ problem that needs some TLC.
Maybe if we started a gang of dentally neglected women and started leaving scary letters in their mail boxes...?

Naw, I'm half-way to the surgery date already. Better to stick it out.
