Frankly, I started simply by making myself a mental health diary - writing about how I felt each day, if I got upset what was the outcome, yadda yadda yadda. I don't know if it's WORKING per say, but I do know I've had one bad day in the last week (which is a definite improvement from three or four bad days a week), so...?
And I've also just started working on my self-esteem, one self-hating thought at a time. I'm still working on: "I can make my friends happy, and I bring something to their lives that wouldn't otherwise be there." I refuse to let myself question that statement anymore and hopefully soon, I'll have accept it as truth easier than I ever accepted "everyone's just talking to me out of pity and no one would care if I died".
I'm also trying to accept myself as I am - if I want to go work out, whether to loose weight or just feel good, I will, but I also want to learn to be happy just as I am, and know that people don't see me as the disgusting blob that I see myself.
You know where I'd start? Make today's list, and clean up your desk area. Small, little things, but you'll have gotten somewhere. Basically, start with the practical things on your list, and deal with the emotional things as situations come up. goldfeesh's idea of a mental health diary sounds helpful, too.
