Usually you buy a monthly membership, which entitles you to a certain number of rentals per month. You don't get to pick out which movies you see when -- you make a list of want-to-sees and they send you stuff off your list as it's available. Kind of fun to be surprised, I guess. *g*

I'm pretty sure you don't pay a rental fee ontop of the monthly fee.
And you can rent them as long as you like and as many as you like.
