I had a similar case of denial with a kidney stone. Around 23:00 I was pretty sure that was what it was, despite never having one before, but I waited until it became unbearable around 04:00. This turned out to be a very bad move since the ER was surprisingly full for a weekday and I spent more than an hour in the waiting room in the worst pain of my life. If I ever feel that again, I am heading out ASAP.
I haven't had one of those before, but to be honest and basing it on what I've heard, you were definitely in more pain that I was.
The little ER I went to first flipped their lid because there were no patients being seen when I got there, so literally 10 minutes before I stumbled in they sent their helo to do a patient transfer and had to wait for another helo to get there to take me to the cath lab. Also, it was a small crew and immediately after they confirmed my MI, two more patients showed up for care.
I haven't commented much in FB on this because I'm having trouble finding words to say. I'm so glad Kailey was able to fly out there. I know exactly how awful this is for the family when you are in another STATE when this sort of thing happens. It's only been a year since Sam was hospitalized in Billings for his scary high blood pressure. That was so horrible and so scary, and I'm grateful every day that he's turned himself around healthwise.
I know you hate vegetables, but maybe you could make it into a manly endurance thing. Find the grossest (and healthiest) foods you can and post pictures of them on Facebook and describe how much you hated eating them. Who knows? You might develop a taste for some of them, and at the very least it would be funny.
That's okay Kim, we've spent enough time around each other that you don't have to verbalize (type, whatever) for me to know what you're saying. I am glad as well that the Mighty Sam has been able to turn around his health, now let's see if I can do it too.
If I could find a few thousand bucks to spare, I would like to get a calcium test or a CT coronary angiography to try to see potential blockage in advance. You would think, with the emphasis on preventive care, insurance would help with that, but no.
There was an "anomaly" detected a couple years ago, and I was sent for a nuclear stress test. It came back fine.
There are places that do full body CT scans that would find things like this. I always wanted to do one just for this purpose, but like you, couldn't afford the outlay since insurance wouldn't cover it.

"So, I go see MM:FR. I have a heart attack. I get a stent.
Or, put in better terms, I saw a kick ass film, cheated death, and came away more metal.
How fucking awesome is that?"

Take better care of yourself!!!! I'm glad you are making quick improvements.
If I had been by myself when my head went 'splody, I doubt I'd still be here. So, props to you for getting yourself to the ER in one piece.
