I'm not positive, but I think that Yidan is referring to the son they had (who was living with them when Jang Geum first came to live with them as a child), who later died...? I was never really clear on that either :P

i didn't watch the starting episodes much, but i heard about the boy who was Jang Geum's friend as a child.

did they show when he died?

No, that's what was so odd about it...suddenly the son was gone and you just assume he's married and left the house or something, but then much later they casually mention that he passed away :( It was slightly confusing

I remembered there was a young boy with them when young Jang Geum showed up. From what I read in other forums the boy died of a disease.

Yeah that's about right. Remember how Jang Geum was treating those small pox patients and stuff later on? I think that was the disease Yidan had, cause they were like... "If Jang Geum was here back then Yidan wouldn't have died".
