Question for my UK friends

I'm drawing a drivers' liscence for one of my British characters and I need to familiarize myself with the British format. Could you guys tell me what pertinent information goes where on the top half?

*RETRIEVES LICENSE*! I only have a provisional, but they're essentially the same besides the colour (mine is green, has a big red L instead of the EU sign and the word 'provisional' stamped all over it

The back of the license is an extended version of the above, also showing how long you're allowed to drive for. (Mine shows my 16th birthday for fkp and my 17th for A and B, up until my 70th birthday for all three - 70 year olds need to renew their licence then and every three years after)
Mine also has information codes on the back - down the right hand column, I have 01 beside every category (showing I need glasses/contact lenses) and at the bottom I have 115,71 (115 - organ donor; 71 - duplicate licence)
