The first time Fin told John that he loved him, they were in the hospital.
It had been after the Ackerman court shooting, after Fin had rushed to the hospital to check on John ,after Agent Lewis had left, and while John was 'happily' drinking his shake (which meant slightly less acerbic bitching about being shot “in the ass!”).
“ I love you.”
“No you don't.”
“ John-”
“Do you have any idea how common it is for a near death experience to provoke that sort of reaction, there's no way I could know that it is how you really-”
So Fin kissed him.

The end of their relationship seemed to cruelly mimic it's own beginning.
It's just the situation that's different.
Time has passed , many years, in fact, and while it has been kinder to them than most, they had both known that what they had wouldn't last forever. They were both cops, and they'd both been married at least once - they knew.
They'd just hoped that it wouldn't be like this.
Two weeks from John's retirement and they find themselves in that damned hospital again, except this time it's Fin lying in the hospital bed and the diagnosis...

The last time John told Fin that he loved him, it was at the cemetery.
