Off the top of my head: Depending on just how broadly you're construing it, pretty nearly everything by Jack Chalker. Memory says that Larry Niven's got some, but memory is fuzzy because I haven't read Niven in years. There's one Bujold short story ("Labyrinth") that would qualify, I think. The Man with Two Brains, a Steve Martin movie (god, I can't believe I remembered that)
And, I have to say, just seeing Blood Music on your list sends a shiver down my spine. That is such a delightfully creepy story.
