How the hell do you tell whether it's an elbow or a knee? I can't even tell which is which if I get one of Kay's in my back at night now.

A knee has more of a kick, and an elbow is just a little nudge, well, a sharp little nudge. To be honest I can't tell all the time, but when you're carrying round a person inside you then sometimes it's pretty obvious :)
