MAJOR SHUTTER ISLAND SPOILERS
It turns out the plot to one of the Disney movies was actually an elaborate role-play scenario invented by the asylum in which they're housed to help them recover from some trauma. What we saw on the screen was just their mentally unstable mind's interpretation of the role-play. The head psychiatrist played the role of the villain to provide a place for the patient to project their problems onto, while the therapist closest to them played their sidekick/comic relief.
