I would have said buttercream initially, but I agree that you should go with structural integrity in this case.

A friend at work suggested just plain gooey caramel in the middle instead of buttercream or cream cheese frosting. What do you think?

If I can manage to make the cake slim enough to pack into a cooler, I could potentially keep the caramel buttercream, however.

I don't really know how much better cream cheese frosting would hold up..caramel buttercream on apple cake just sounds more yummy to me. But I'm sure it will taste good either way?

Cream cheese frosting is my vote... it might help cut down the intense sweetness of caramel icing. I know that even I find caramel icing very sweet, and I have an uber high tolerance. ;)
