Put sticky notes everywhere with little notes that remind you of what you should be doing the night before. Like on the T.V. write yourself a sticky note that says, "Go to gym." On the shower stall put one that says, "Take a 10 minute shower," and on the way out put a note on the door that says, "Eat Better." Personalize them, they will remind you of your resolve the night before. Go to the gym even if you step into the door for two minutes, and most importantly keep at it until you start going every morning. You'll feel better and that is enough of a reward for anyone. But reward yourself with a treat and soon you'll be on your way.
