It's a nitpick, but I have too. From the character info:
"Doctor Rodney McKay: Neurosurgery is McKay's life. He has no bedside manner what so ever, and would love to be able to never see the patients until they have been knocked out by Anesthesiology."
When ever possible, brain surgery is done with the patient awake. I LOT of time is spent zapping the brain to see what a particular part does. This is done to try to minimize the chance of cutting something important (like speech, or sight). You might have seen that House ep.? It's true. Local anesthesia the patients get; general, not so much.

Yes, I do know that those surgeries are done awake...it was more along the lines of trying to show McKay's personality when I wrote that. :)
It actually comes from a real Neuro-doc that I know who always says that he wishes he wouldn't have to actually talk to his patients. That they could all just be knocked out so he could do what he pleased without worrying about being nice.
I figured that if anyone truly was going to go deep into brain surgery, they would do their research and see that they are mostly awake. I also figured that if noone wanted to go that deep, it was still fun to play around with how much of a jerk McKay really is. :)
