We use these types of rooms at the hospital where I work. If they are in time out, which is voluntary, meaning they walk in without hands on and the door is not locked, they are watched continuously on camera. If hands on is needed, it's considered seclusion. The door is locked, they are watched by a person sitting at the window of the room for the first hour, with that person making notes every 15 mins, then any time after that is monitored on camera. Plus, if we know they are a danger to themselves then anything that can harm them is taken away and they may even be put in what's called strong clothes, so they can't possibly choke themselves.
