You lose the game if you play your combo. Here's why:
Phage's text says "When CARDNAME comes into play, if you didn't play it from your hand, you lose the game."
'Playing' a spell or card in Magic refers to when you pay the casting cost to put it on the stack. Using Sneak Attack on Phage doesn't involve 'playing' Phage by this definition - Phage is not a spell, but is instead put directly into play with Sneak Attack's effect.
When Phage comes into play, her triggered ability checks to see if she was 'played' as a spell from your hand. She wasn't, and you immediately lose.
Bah, good point. You just reminded me of the reason why I wanted Angel's grace so much for a red/white deck with some Phages. ;) (W, instant. Split-second. You can't lose the game and your opponent can't win the game this turn, and any damage that would take your life total below one drops it to one instead). If I play that, the triggered ability is activated, but due to Angel's grace, I can't lose the game

Or just put her into play on your opponent's side. That works too.
