I would let him know that if contacted about him, you would have to mention the fact that he has had to drop out for inexplicable reasons. And if he was comfortable with knowing that you would mention that, you are happy to be a reference.

Especially since you could honestly say that when he is there, he is great. One black spot on an otherwise glowing recommendation lends it honesty and weight as far as the caller is concerned.

I think it would be fair to let him know though, that professionally you would feel obligated to mention that he doesn't always follow through.
