Some Hurt/Comfort Shizaya with Izaya on the hurt side and Shizuo on the comfort side:
Something pretty fucked up happened to Izaya and due to that, something's wrong with him (more than usual, that is). Lots of people know about it but Izaya, being his slightly psychotic self, doesn't really realize he's gone and cracked a little more than usual. (Unintentional/unnoticed self harming tendencies on Izaya's side?)
Shizuo is asked by someone (either Shinra; or someone random worrying that Izaya might snap completely and Shizuo would be the only one really capable of stopping him)
I beg you, dearest, wonderful durarara!kink - gimme some Izaya!Hurt!
(Hell, why do we always love to see the characters we like suffer so much XD ?)

OMG I really want this!! Yes, I feel the same like you too,anon. I also like my fav characters suffering(especially the uke)and then gets comfort from the seme.;)
It's also interesting to see Izaya's cracking up cause he usually totally in control of everything.

It was very late at night. Only the artificial lights of garish neon warded of the deep darkness that was not yet on the edges of dawn. An insidious red glow invaded Shinra’s office and gave him an uneasy feeling. There were times when he resented not having a license and being forced to work in shady places opposite of illegal casinos, strip joints and brothels. This was such a time. These last hours of the night often felt warped to him. It was of course highly unscientific of him and if asked under the sun Shinra would laugh off such superstitious of silly. After all he could not afford such niceties. Not in his world where health regulations were mere general guidelines. Every now and then his alcohol came from whiskey bottles. Shinra waited until Izaya was swept away into induced sleep before he disinfected and bandaged his wounded hands. It was so much like the good old days but utterly different. Distorted, even. Tending to unconscious patients was routine to him but not when they were friends. Shinra adjusted his glasses and took a deep breath before calling Celty over. He needed her to make sure that Izaya did no more harm to himself

I have experienced something similar to the general idea of this fic so I thought I'd be better qualified to write about it than most. Which is not at all a good thing but it was why I picked it despite already having far too many fics in progress./rant

(OP) I really, really love you for this. It's really heartbreaking, but that's just a small part of it - the whole story has such a strong sense of unease to it that it's nearly tangible, the whole atmosphere is kind of suffocating (I may ramble strange things now, I hope it doesn't come across as too ridiculous) - it's all very, very credible. In a very creepy way.

This anon has personal experience with the way Shizuo is feeling right now - that's a scary situation, and you feel alone and completely helpless to do anything about it, for yourself or the hurting patient. And sometimes there really is just nothing you can do, and that's what hurts and scares you the most.

This is very well written. I don't think I'd be able to put those feelings down in a coherent fashion, and I respect that in those few lines at the very end with Shizuo you captured that spreading feeling of absolute dread.
