The Emperor definitely killed Vader, either through the malfunction of his machinery, or simply causing too much damage to his old body to where he couldn't take it anymore.

And because the young Anakin is at the end there, one might argue that you appear either a) how you want to be seen (at this point he's regretted his turning to the dark side) or b) how you last were before dying or turning to the dark side. We might infer that the ghostly speech and guidance from beyond the grave is a light side ability of some kind that Qui-gon figured out.

I think the story that the Emporer told about the Sith who was able to keep people alive who should be dead and was later killed by an apprentice who was taught the technique was a reference to himself. You could say that he used it only as a manipulation tool to make Anikan turn, but there is too much inferences that make my beliefs correct. When the Emperor rescues Vader at the brink of death in episode 3, he seemed that it was okay that he was dying, as long as he wasn't already dead.

After reviewing episode 6 again, I think another hint is when Vader and Luke walk through a corridor and Vader mentions Luke's new Lightsaber, Luke tries to get him to turn away from the Dark side and Vader mentions that Luke did not fully understand the power of the Dark Side, probably inferring to the ability the emperor had to keep him alive.

It may have been Palpatine that actually killed his master (who knew of these live-preserving abilities), but the point is moot. This is because we know that Palpatine doesn't know the secrets to keep people alive, or bring people back from the dead. He says as much to Anakin once he turns to the dark side, and it's already too late.

And any number of things could be meant when Vader tells him he doesn't know the full power of the Dark Side. Your idea is just clouding your vision and you're trying to bend circumstance to it. Vader could have just as easily been talking about the ability to cast lightning, because at that point Luke had no idea such a thing was possible.
